Provide the customer’s contact information, current medications, medication schedule, and any optional health or emergency information. Select the Basic or Premium package and indicate whether you would like a printed copy mailed to you.

After submitting the form, you will receive a separate email containing a secure CareReady RX payment link. Submitting the intake form does not mean you have paid.

Open the payment link and complete your payment using one of the available payment methods. Your order will be processed after payment is received.

4. Receive Your Personalized Documents

CareReady RX will prepare your personalized Medication Checklist. Premium customers will also receive a Personal Health Summary. Your documents will be delivered by email, and a printed copy will be mailed if that option was selected.

5. Optional Updates

If your medications change, I can update your checklist anytime, so it stays accurate and helpful.
